Dear Parent/Guardian:      Thank you for your interest in enrolling your child to attend Clayton Youth Enrichment Services. The following process must be completed before your child can be accepted into our program. On-Line Enrollment has ended. You  may pick up and complete enrollment paperwork at the center on child's first day of attendance. or contact our Client Service Department (817) 923-9888 to request an enrollment packet. You must complete ALL of the required enrollment forms. The Enrollment Fee must be paid at time of enrollment. Payment is part of the On-Line Enrollment process. Fees for 2010-2011 School Year Your child is not considered enrolled until all Enrollment Forms have been reviewed and processed and payment has been received by our Client Service Department. This process can take up to 5 working days to complete. If your child is taking medication on a regular basis, has a medical diagnosis, is in special classes at school, or has a physical disability, please call our Corporate Office at 817-923-9888 for an appointment to assess the needs of your child. For more information, refer to Assessments section of the 2010-2011 Parent Handbook (see below). For your convenience we will accept the following forms of payment: Credit Cards/Debit Cards – Visa, Mastercard, American Express. Checks and money orders – made payable to Clayton Youth Enrichment Services   REMEMBER:  Our policy states that center staff will not accept payments at the center after the initial enrollment.   All questions concerning billing and collection of fees should be directed to the Client Service Department. This will allow our center staff to focus on the needs of you and your child.   Sincerely,   Robert E.
